Error Avoidance Program
The programme is designed to assist health care organisations in the
avoidance of adverse incidents. The specific aim is to reduce the risk
of harm to patients through the promotion of a teamwork approach to
improving patient safety and quality of care. The programme is based on
concepts developed and proven in commercial and military aviation. It
develops proficiency in Error Avoidance techniques, via a combination of
formal instruction and facilitated workshops.
An overall objective of the programme is to equip the organisation with
knowledge and skills to internally maintain an Error Avoidance
Programme.
Outcomes
The outcomes of the programme are:
- A proactive approach to error avoidance
- Awareness of the elements of incipient
error chains and ability to implement effective intervention
- Greater adherence to standard operating
procedures (SOPs), protocols and guidelines
- Integration of teamwork and error
avoidance into the day-to-day working of the organisation
- A cultural change that allows for sharing
and learning from experience and mistakes through more openness and
increased communication
- Consideration of methods to actively
promote awareness and understanding of adverse patient incidents to
all individuals directly or indirectly involved in patient care
- Improved knowledge of the potential
impact of human performance limitations and behavioural attitudes
and values on patient safety
- A team with the knowledge and skills to
internally maintain the Error Avoidance Programme

“As a conclusion it is clear
that this process would reduce errors, cut
the costs of litigation, reduce wasted capacity
and reduce the patients stay in hospital”.
A report of three projects used to
demonstrate the transferability of aviation’s
human factors approach to patient safety in
healthcare organisations.
The projects were
commissioned by the National Clinical Governance
Support Unit of the NHS Modernisation Agency
with the specific aim of identifying the
strengths and weaknesses of the Team Resource
Management approach used in aviation and its
influence upon the attitudes and behaviour of
teams of front-line clinical staff to patient
safety. |
